On your behalf, after services are provided we will file claims to your insurance company. We will assist you in making sure that your insurance pays us timely and appropriately for services we have provided to you. However, please know that ultimately you are financially responsible for payment of bills incurred.
PAYMENT DUE AT TIME OF SERVICE
Cancellation Policy:
All sessions must be canceled/rescheduled 24-hours prior to scheduled appointment time to avoid a $70 cancellation fee. If you do not cancel/reschedule your appointment with 24-hour notice and you are assessed a $70 cancellation fee, this fee must be paid in full prior to the next kept appointment.
Returned Check and/or Declined Card Policy:
There is a $35.00 charge for returned checks and card declined fee for any credit/debit transaction that does not go through.
